At myHomecare we understand there may be times in our life when we may need a little help. This may be to get the day started, getting out and about or help around the home. CHSP works by maximising independence through an individualised and enablement-focused range of services and supports to enhance your quality of life in your own home. As a leading provider myHomecare has been approved by the Government to deliver subsided care and services.
Eligibility for this program is based on your support needs and age. If you’re having trouble with everyday tasks and feel that a little support could improve your health and wellbeing, CHSP could be right for you.
To be eligible for this program, you must be aged:
A face-to-face assessment will determine eligibility and ensure that the right services are made available to those that need them. If your needs are more complex, a Home Care Package could be better suited to you.
Services that keep me well and independent
Services that keep me connected with my community
Services that keep me safe in my home
After you have been assessed by myAgedcare you will be provided a reference code which we can use to access your assigned care. Depending on your individual needs will arrange to complete an in-home assessment with one of our Care Managers as soon as possible to determine how your care will be delivered. You can expect to commence services within 2 weeks of your in-home assessment by myHomecare.
If you are eligible, you’re expected to contribute to the cost of your care if you can afford to. The cost depends on the type of support and the provider. Simple services like house cleaning and meals might cost a few dollars. More complex services like renovation work will cost much more. Please call us to discuss your individual needs and determine how much you may need to contribute.
In accordance with the myHomecare fees policy, no client who is genuinely financially disadvantaged is refused service.
myHomecare services are planned to meet the individual needs of clients. Care Plans are developed with consideration for a client’s specific language, cultural and religious requirements. myHomecare maintains close working relationships with local Veteran, Dementia, CALD and ATSI communities. If you would like to discuss your particular requirements please contact us.
