Home Care Package (HCP)

At myHomecare we understand there may be times in our life that we may need a little help. This may be to get the day started, getting out and about and help around the home. A Home Care Package can support you with a range of complex care needs to live independently in your own home.

How does it work?

A Home Care Package uses a consumer-directed care approach to make sure the support suits your needs and goals, aimed at enhancing your quality of life in your own home.

There are 4 levels of Home Care Packages — from level 1 for basic care needs to level 4 for high care needs, using a coordinated mix of services that can include: help with household tasks, equipment (such as walking frames), minor home modifications, personal care and clinical care such as nursing, allied health and physiotherapy services.

As a leading provider of Home Care Packages, myHomecare has been approved by the Government to deliver subsided care and services to older Australians.

 

Who can access a Home Care Package?

Eligibility for this program is based on your support needs and age. If you’re having trouble with everyday tasks and feel that ongoing help and support could improve your health and wellbeing, a Home Care Package could be right for you.

To be eligible for this program, you must be aged:

  • 65 years or older (50 years or older for Aboriginal or Torres Strait Islander people), or
  • 50 years or older (45 years or older for Aboriginal and Torres Strait Islander people) and on a low income, homeless, or at risk of being homeless.

A face-to-face assessment will determine eligibility and ensure that the right services are made available to those that need them. If your needs are more complex, a Home Care Package could be better suited to you.

 

What HCP services can myHomecare provide?

Services that keep me well and independent

  • Meals and other food services: Pre-made meals and help with food preparation
  • Personal care: Help with showering, self-care, hygiene and grooming
  • Nursing: Help with medicines, telehealth support
  • Allied health & therapy services: Health and therapy services e.g. podiatry and occupational therapy
  • Specialised support: Help for people with a particular condition e.g. vision, dementia
  • Respite care: Care for you while your carer takes a break

Services that keep me connected with my community

  • Transport: Community transport or vouchers to attend appointments or activities
  • Social support: Contact and company with others e.g. social groups

Services that keep me safe in my home

  • Domestic assistance: Help with basic chores around the house
  • Home maintenance: Help with keeping your home and garden safe
  • Home modifications: Changes to your home to improve safety and access e.g. ramps and rails
  • Goods, equipment and assistive technology: Items to help you get around or adapt e.g. walking frame, shower chair

 

How soon will I have to wait until I can get some help at home?

After you have been assessed and approved by My Aged Care you will be placed on the national queue for your Home Care Package to be assigned.

The time you will have to wait to be assigned from the time you are approved for a package on the national priority system, is based on:

  • your assessed priority for home care (either medium or high)
  • the date of your approval

There is a lot of demand for Home Care Packages, so you may need to wait a while before My Aged Care have a package available to assign to you. It’s important to contact myHomecare to discuss your options. You might be able to access interim services whilst you wait, so you can get the support you need to remain living at home whilst you wait for your HCP – just give us a call and we can talk you through your options.

 

Is there a cost?

The total amount of funding in a Home Care Package budget is made up of:

  • the Government subsidy, including supplements for your specific care needs once eligible
  • basic daily fees might be payable depending on your situation
  • income-tested care fees, payable by people who have income over a certain amount
  • amounts for additional care and services, payable if you want extra services that the package funding would not otherwise cover

The Government and the care recipient pay these amounts directly to the provider.

In accordance with the myHomecare fees policy, no client who is genuinely financially disadvantaged is refused service.

 

How do we meet our client’s special needs?

myHomecare services are planned to meet the individual needs of clients. Care Plans are developed with consideration for a client’s specific language, cultural and religious requirements. myHomecare maintains close working relationships with local Veteran, Dementia, CALD and ATSI communities.
