The good news is that myHomecare is able to work with you and manage most of the process of switching for you. All you have to do is let us know by calling 1300 20 90 20 and we can guide you through it.

If you choose to switch to a myHomecare provider, we work with your existing provider and communicate with My Aged Care to transfer your package so that you don’t have to worry. We’ll then ensure your new provider delivers the services you need from day one and every day after that.

As you can see, by getting in touch with myHomecare, there’s no need to further delay changing your Home Care Package provider. Simply give us a call and we’ll do the rest.

How can I access more care?

We understand at the time of your assessment, the program that was provided to you was expected to meet your current needs, however over time you may have identified that your individual circumstances may have changed and you now require a little more long-term help to continue living independently.  If you feel that you may benefit from ongoing support, a home care package is only a phone call away. myHomecare will work with you to arrange for a new assessment with My Aged Care to determine if this is best suited to you. If a Home Care Package isn’t what you need, additional support is also available such as other programs which can also assist.

These may include Transitional Care, Short Term Restorative Care, Commonwealth Home Support or Private Care.

My needs have changed, can I be reassessed?

Your care needs may change over time. If this happens, you can change your services to better suit you. If you notice that you or a loved one’s care needs change, call us at myHomecare in the first instance and ask for a review of your care plan.  You may need a different combination of services to meet your new needs. We will work with you to review your care plan and to package your budget. Following the initial review, your Care Manager may suggest the need to be reassessed for a higher level Home Care Package or other support services. Together with your Care Manager, the myHomecare team and My Aged Care can help to arrange for this to be done with another in-home assessment by a qualified ACAT team member.
