My Aged Care will ask you a series of questions regarding your personal details such as your name, date of birth, address, next of kin and also questions to determine the level of support you potentially need.
The following items are the things you’ll need with you for your call with My Aged Care.
You get a Medicare card when you’re enrolled in Medicare. My Aged Care will ask you for the number, your position on the card and expiry date.

View example of a doctors refferalHave your Income Tested Care Fee Form filled out and ready for your call.
View example of this formIf a family member, partner or carer be involved in your care decisions, have this ready for your call.
View example of this formDoes someone have Power of Attorney or Enduring Power of Attorney for your affairs?
If yes, then they will need to be on the call with you when you register.
