My Aged Care won’t develop an individual care plan or organise your local services you need to remain healthy and independent at home. This is done from your local provider in which you choose to engage with for home care services

My Aged Care will make an appointment with you to meet one of their assessors – these are sometimes called RAS (Regional Assessment Services or ACAT – Aged care Assessment Team). They will come to your home, sit down with you and will ask you a series of questions regarding your personal details such as your name, date of birth, address, next of kin to verify who you are.
Then they will work through a series of questions and capability observations that help them determine the level of support you require. It is important to be prepared for this appointment with some documents and information.
