Transitional care helps you recover after a hospital stay. It provides short-term specialised care and support to help you regain your functional independence and confidence sooner, and avoid the need for longer term care and support services. Your care is tailored to your needs and goals, and is delivered in the most appropriate place to help you meet them. This could be in your own home, out in the community, or a mix of these locations, as your needs change with your recovery.
Referrals may be made from most public or private hospitals to ACAT to assess your eligibility for a TRANSPAC. You can ask the hospital staff to arrange this face-to-face assessment. The assessor will then send your referral to myHomecare for us to assess the suitability for your transitional care.
If eligible, we will then assist you with leaving the hospital, as well as entering and exiting care.

You can access transition care services for up 12 weeks per year. There are possibilities for extensions for another six weeks, if required, but this involves further assessment.

The hours per week and frequency of visitations offered to a potential TRANSPAC client will depend upon the person’s assessed needs and myHomecare program resources.
The frequency of your visits will be regularly reviewed as your independence is regained.
The TRANSPAC program guidelines require clients to contribute to the cost of service delivery. Clients contribute a weekly program maintenance fee that is based on 17.5% of the maximum basic rate of pension.
In accordance with the myHomecare fees policy, no client who is genuinely financially disadvantaged is refused service.
myHomecare services are planned to meet the individual needs of clients. Care Plans are developed with consideration for a client’s specific language, cultural and religious requirements.
